All consumers are entitled by law to request a free annual medical report file from each of the nationwide specialty consumer reporting agencies. The three major specialty consumer reporting agencies for medical information are (1) the Medical Information Bureau, Inc. (MIB, Inc.), (2) OPTUMInsight, Inc. (formerly Ingenix, Inc.), and (3) Milliman, Inc.

Free Annual Medical Report Request Options

Consumers may request their free annual medical report by telephone or by submitting a written request to each specialty consumer reporting agency. Here is the information you need to submit your free annual medical report request:

(1) The Medical Information Bureau, Inc. (MIB, Inc.)

To make your free annual medical report request from the Medical Information Bureau, Inc. (MIB Inc.), complete one of the following options: (1) Call the Medical Information Bureau, Inc. toll-free telephone number at 1-866-692-6901 to make your request. OR, (2) Download this free Medical Information Bureau, Inc. consumer request form. Then mail the completed and signed form to “Medical Information Bureau, Inc. – Consumer Request, 50 Braintree Hill Park, Suite 400, Braintree, MA 02184.”

(2) OPTUMInsight, Inc. (formerly Ingenix, Inc.)

To make your free annual medical report request from OPTUMInsight, Inc. (formerly Ingenix, Inc.), complete one of the following options: (1) Call the OPTUMInsight, Inc. toll-free telephone number at 1-888-206-0335 to make your request. OR, (2) Mail a written request to “OPTUMInsight – MedPoint Consumer Request, 2525 Lake Park Boulevard, West Valley City, UT 84120-8230.”

(3) Milliman, Inc.

To make your free annual medical report request from Milliman, Inc., complete one of the following options: (1) Call the Milliman, Inc. toll-free telephone number at 1-877-211-4816 to make your request. OR, (2) Mail a written request to “Milliman, Inc. – IntelliScript Consumer Request, 15800 W. Bluemound Road, Suite 100, Brookfield, WI 53005-6043.”

Delivery Reminders

Two important reminders: (1) If necessary, the consumer reporting agencies may contact you directly by telephone to verify your identity or confirm your request for disclosure. (Failure to respond could delay the delivery of your disclosure files). (2) Within 15 business days of receipt, each consumer reporting agency must individually provide your medical report disclosure file to you by US Postal Service mail.

History of FTC Regulation

Each of these specialty credit reporting bureaus has been the subject of investigations by the Federal Trade Commission regarding the disclosure of free annual medical reports to consumers. Under an agreement with the Federal Trade Commission in 1995, the Medical Information Bureau, Inc. (MIB), and all insurance companies who are members of the Medical Information Bureau, Inc. (MIB) agreed to abide by the Fair Credit Reporting Act (FCRA).

Under an order from the Federal Trade Commission in 2008, OPTUMInsight, Inc. (formerly Ingenix, Inc.), and all healthcare and insurance companies that utilize the MedPoint prescription profiling technology, agreed to abide by the Fair Credit Reporting Act (FCRA). Under a similar 2008 Federal Trade Commission order, Milliman, Inc. and all healthcare and insurance companies that utilize the IntelliScript prescription profiling technology, agreed to abide by the Fair Credit Reporting Act (FCRA).
